Required Documentation

Within six months of your wedding, please contact the parish where you were baptized and arrange for your Baptism and Confirmation certificates (referred to as “a long baptism certificate”) to be sent directly to the parish where you will be married, with attention to the priest or deacon who will be performing your marriage:

St. Martha Parish, P.O. Box 1745, Plainville, MA  02762


This documentation is necessary to complete the marriage forms and the required sacramental records.
