CORI Forms Are Due

If you are involved with parish ministry, please stop by the office to fill out a CORI form. This includes Eucharistic Ministers, St. Vincent DePaul volunteers, Lectors, Cub Scout Leaders, Ushers, and Knights of Columbus.

If you cannot get to the office, CORI forms are located on our website. You may fill out the form and have it checked by parish staff.
