Annual CORI Form– All Volunteers and Employees

It’s that time of year again when all parish employees and volunteers (18 years of age or older) are required to complete a CORI and Volunteer Emergency Contact form.

The CORI form is an annual requirement which is intended to help protect our children, youth, elderly and disabled. The Volunteer Emergency Contact Form is designed to help our Collaborative know who to contact in case of an emergency for our volunteers.  Also available for review is the Code of Conduct policy.

For your convenience, you can complete a CORI form and Emergency Contact form at

St. Mary on Sunday, March 26 after the 9:30am Mass or at

St. Martha on April 2 after the 8:00am Mass downstairs in the Hall.

Only one CORI and Volunteer form needs to be submitted to either parish regardless of your ministry or where you are registered. If you can’t make one of these times, you can always complete forms at either parish office or download forms (links highlighted above) from our website pwc.church and return completed original forms to the parish office or a Clergy member after Mass. The original CORI form is validated with a license or state issued ID. Thank you!
