CORI Form Renewals

If you are involved with parish ministry (this includes Eucharistic Ministers, Lectors, Music Ministers, St. Vincent de Paul volunteers, Scout Leaders, Knights of Columbus members, etc.), please fill out and submit a CORI Form for renewal in 2021.

CORI forms are available on our website, pwc.church, under the “Contact” tab, as well as in our parish offices. Please bring your completed form to your parish office for review and submission.
